Authorization and Consent Release Form

I fully and freely authorize/consent to the use and reuse of my name, photographs, images, recordings, and/or comments (“Works”) by Russell Sage College (RSC) in any advertising, promotion, or marketing on RSC’s web site, RSC social media (including but not limited to Instagram, Facebook, and LinkedIn), on television or radio, and/or in any print or digital publications.  I do hereby release and hold harmless RSC and its employees, Trustees, agents, and assigns from any liability arising out of the use of the Works provided in this Authorization and Consent.  

I understand that RSC may modify the Works and owns all variations of the Works, and I waive all rights to any payments and/or royalties in connection with the use of the Works.

I have read this authorization and consent before completing this form and warrant that I fully  understand the contents.
